What to Bring to the Appointment

• Patient's insurance card
• Physician referral forms and authorizations if required by insurance
• Pertinent information about your child's medical and surgical history
• Any recent diagnostic imaging such as x-rays, CT, or MRI scans you may have

Insurance and Payment Information

Our physicians are providers for most insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility.

We request payment at the time of the appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of the appointment. When necessary, our staff will work closely with families who require a payment plan.
